Licensed Practical Nurse ( Private Home Care)

Job Summary

We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to provide exceptional private home care services. In this vital role, you will deliver personalized nursing care directly in clients’ homes, ensuring their health, safety, and comfort. Your expertise will support individuals with diverse needs, including seniors, those with chronic conditions, and individuals requiring specialized care such as dementia or post-acute recovery. Duties

• Conduct comprehensive patient assessments, including vital signs, physical examinations, and health histories to develop tailored care plans.
• Administer medications, injections, IV infusions, and other treatments following physician orders and safety protocols.
• Monitor patients’ conditions closely, recognizing signs of acute pain or deterioration and triaging effectively for urgent medical attention.
• Document all patient interactions accurately using Electronic Health Record (EHR) systems like Epic or Cerner while maintaining HIPAA compliance.
• Educate patients and their families on health management strategies, medication administration, and post-discharge care plans.

Qualifications

• Valid Licensed Practical Nurse (LPN) license in good standing with the state licensing board.
• Proven experience in private home care or related settings such as assisted living, nursing homes, or hospitals.
• Strong knowledge of medical terminology, anatomy, physiology, and diagnostic evaluation techniques.
• Familiarity with advanced nursing skills such as, catheterization, sterile technique, and medication administration.
• Excellent patient assessment skills combined with critical thinking to prioritize urgent needs effectively.

Job Types: Full-time, Part-time, PRN, Per diem, Temporary

Pay: $23.00 - $25.12 per hour

Expected hours: 24 – 96 per week
